Update on Household Income Form
Even with universal meals, public and independent schools are still required to report student level household income information via school census reports (DC06, DC04 and DC03, or the Independent School Census). In addition to direct certification information, schools participating in the National School Lunch Program (NSLP) who are operating in a ‘base’ year of Provision 2, gather student level household income information from their school meals applications. For schools in a non- ‘base’ year or those not participating in the NSLP, the Agency has created the Household Income Form as a helpful tool to collect this specific census data. These data are used for a variety of educational purposes including calculation of pupil weights in the funding formula, determination of the “equitable share” of Title I resources to be set aside for services to independent schools, and educator loan forgiveness qualification.
The Household Income Form is provided in both a PDF format and an online format. Both versions along with FAQs such as: which form to use under which circumstances and how to report income status in data collections are available on the HIF website, Household Income Form webpage. In addition, the Agency will again provide “Fill the Form” materials for parent communication to encourage families to complete these forms. Data Town Hall meetings were held in early Spring for registrars, business managers and data managers. The next Household Income Form Data Town Hall meetings are scheduled for August 27th and a special September 10th meeting for Independent Schools. The meeting invitations have been sent to your SU Data Managers/Independent Census contact person to share with data staff.
